WHAT IS HOME HEALTH CARE?

Home Health Care is a service provided to assist in a person’s recovery from an illness, accident, surgery, or a change in medical condition. Professional healthcare and rehabilitation services are provided to the patient in the comfort of their own home until recovery goals have been met and while under the direction of their Primary Care Provider.

WHY DOES MY DOCTOR ORDER HOME HEALTH CARE?

Your Medical Provider will prescribe Home Health Care services when the skills of a healthcare professional such as a registered nurse or therapist are needed to assist you with recovery from an illness, injury, hospitalization, surgery or to help manage a chronic condition which requires medical attention.

Do I need Home Health Care

Qualifications for Home Health Care Services

HOME HEALTH SERVICES PROVIDED

Skilled Nursing

Home Health nurses provide direct care and health education to the patient and their caregivers. The Home Health nurse is responsible for observing, evaluating the patient’s plan of care.

Physical Therapy

Physical Therapists work with the patient to prevent further injury or physical deterioration and aim to the restore patient back to baseline. The therapist will develop a comprehensive plan for each patient to help restore or improve their physical condition due to surgery, injury, or decline due to illness or disease.

Occupational Therapy

Occupational Therapists assist patients in restoring or enhancing their ability to perform their tasks of daily living. The goal is to achieve the highest level of functioning possible.

Speech Therapy

Speech Therapists work with the patient to restore, improve, or fine-tune essential skills needed for daily life such as swallowing, processing information, & communicating.

Home Health Aides

Home Health Aides mainly assist patients with activities of daily living (ADL’s) which may include:

Social Worker

Social Workers provide counseling and patient advocacy. Our social workers assist patients in accessing services that are available to them based on their insurance provider and available community, state, or even federal resources.

WHO PAYS FOR HOME HEALTH CARE?

Home health care cost

If you are Medicare eligible and meet the Home Health qualifications, there are no out-of-pocket costs for your home health care needs.

Private Insurance will pay for Home Health Care services. Benefits vary per policy and verification of benefits is required. Co-pays may apply.
